Chronic Low Back Pain With Sacroiliitis and L5-S1 Bulging Disc

I have been seeing this patient for some time who suffers from chronic low back pain.  The magnetic resonance imaging (MRI) findings show facet arthropathy and a bulging disc at the L5-S1 level.  She also has sacroiliitis and piriformis syndrome.  The pain radiates from the low back into the back of the leg to the knee and then travels into the foot (radiculopathy).  The knee feels like it is going to give out at times.  I first performed sacroiliac joint injections (sij) as I felt this was her primary pain generator and the pain did improve for several weeks, but it soon returned.  I have also performed piriformis injections and again her pain improves.  This suggests that multiple areas are causing her pain.  Because the pain did not improve an MRI was done, which showed the L5-S1 bulge and I performed a lumbar epidural steroid injection (LESI) at L5-S1.  However, the pain did not improve and I changed the injection to a transforaminal steroid injection (TFB) at L5.  The patient was referred to physical therapy but she has not improved as much as I would like and although I do not think surgery is indicated I have referred her for an opinion from the neurosurgeon so that I am not overlooking something.
